Getting Started

An Acacia Home Care representative will meet with the patient and his or her family and perform a in-home evaluation.

Acacia Home Care will contact the patient’s physician to determine the individual’s specific home care needs.

Based on the patient’s diagnosis and prognosis, specific care needs, home environment, amount of family support, and available resources, Acacia Home Care will develop a comprehensive plan of care.

An estimated cost for proposed services will be provided.  In addition, Acacia Home Care will conduct a thorough insurance investigation to determine all available benefits.

Acacia Home Care will then select a team of healthcare professionals who will implement the specified home care program.

An Acacia Home Care Registered Nurse will make periodic visits to monitor treatment and to make any necessary revisions of the home care plan.
